As a Direct Primary Care (DPC) practice and a Covered Entity under the Health Insurance Portability and Accountability Act (HIPAA), Raphara Health is required by law to maintain the privacy of your Protected Health Information (PHI) and to provide you with notice of our legal duties and privacy practices.

A. How We May Use and Disclose Your Health Information (Standard Care)

  • Treatment: We may use your PHI to provide medical care, including sharing it with specialists, labs, or pharmacies you utilize.

  • Payment: We may use your PHI to process membership payments, coordinate with Health Shares, or verify coverage if applicable.

  • Operations: We may use your PHI for internal quality assessment, training, and licensing activities.

B. Uses and Disclosures Required by Law (No Authorization Needed) We may use or disclose your PHI without your prior authorization in the following specific situations required by state or federal law:

Complaints If you believe your privacy rights have been violated, you may file a complaint with Raphara Health's Board of Directors at admin@raphara.health or with the Secretary of the Department of Health and Human Services (OCR). You will not be penalized or retaliated against for filing a complaint.​
